Required before practice/tryouts:
1. Electronic Forms - ZCS has partnered with FinalForms to collect physicals and online paperwork for student-athletes. FinalForms allows you to complete and sign athletic participation forms for your student-athlete digitally. You may review your data at any time to verify its accuracy. You will be required to sign your forms once per year, prior to your first practice. You must also upload the doctor's signature page of your Physical Form directly to your FinalForms account.
Please register at https://zionsville-in.finalforms.com and follow the prompts to create your account, create your students and sign your forms.

2. Physical Form - Must be on the IHSAA physical form (see below). Page 3 must be uploaded to FinalForms. In order to be valid for the 2023-24 school year, physical examination must be completed on or after April 1, 2023.
Required before first contest:
Program Support/Transportation - $50 per sport. No form is necessary. Checks should be made out to ZCS and can be turned in to the athletic office or mailed to the address below.

ImPACT Testing:
Baseline ImPACT testing is no longer required for student-athletes. It is NOT necessary to schedule an individual appointment to complete the ImPACT test.
