Ascension St. Vincent Sports Performance is grateful for the opportunity to continue serving Zionsville Community Schools, its student-athletes, coaches, and families. While we continue working to fill the full-time athletic trainer position at Zionsville West Middle School, our team remains committed to providing exceptional sports medicine services, including injury evaluations in the athletic training room and athletic trainer coverage for practices and competitions.

All injuries and treatments will be documented in Healthy Roster, a HIPAA-compliant electronic medical record (EMR) platform used by schools and athletic organizations to improve communication between athletic trainers, physicians, coaches, athletes, and families.

We ask all families to register their student-athlete in Healthy Roster and ensure the following information is complete and up to date:

Legal name

Date of birth

Parent/guardian email

Parent/guardian phone number

ZWMS graduation year
